1. May 26, 2012 Truck movement before approved times as per haul agreement # 2012-H-08. As this was a second infraction, the suspension is one week
2. May 26, 2012 Failure to provide adequate dust control as per haul agreement # 2012-H-08. This was a second infraction as such the suspension is five days
3. May May 29, 2012 Failure to provide adequate dust control as per Haul agreement 2012-H-08. This was a third infraction as such the suspension is one month.
